This protocol is for all acute coronary syndrome patients with or without thrombolytics or glycoprotein IIb/IIIa agents and when lower range anticoagulation is desired.
Initial bolus is 60 units/kg to a maximumm of 4,000 units, calculated in Epic.
Initial dose is 12 units/kg/hour to a maximum of 1,000 units/hr, calculated in Epic.
Subsequent goal aPTT is 53-71 seconds or heparin anti-Xa 0.2-0.5 units/ml.
The first aPTT is obtained 6 hours after initial infusion is started.
Once 2 consecutive aPTTs are between 53-71 seconds, repeat aPTT in am daily.
Notify physician for 2 consecutive aPTTs < 53 or > 71 seconds.
